Our staff is
trained to make your child's first visit a
pleasurable one. Some children may be
fearful, but once a child becomes familiar
with the new surroundings, the fear
disappears. To prepare your child for the
visit, be positive in your approach and let
us explain to him or her what will happen
during the appointment.
What Should I
Expect on a First Visit?
Your child should have their first visit
once their first primary tooth erupts and
not later than their first birthday. This
first visit focuses on establishing a
"dental home." During this visit, Dr.
Azadpur will discuss teeth eruption
patterns, oral hygiene, fluoride concerns,
nutrition and diet, any habit (pacifier,
thumb sucking, etc.), dental sealants and
interceptive orthodontics. She will
then plan a re-care schedule to fit your
child's needs. Older children will receive a
thorough examination, necessary dental
x-rays, cleaning and a fluoride treatment.
What Should I Bring
to a First Visit?
After you call to make the first appointment
for your child, we will send you an
informational packet in the mail or you may
click on the link below to view and print
the necessary form(s). Please complete
the Medical History, HIPPA, and the
financial agreement forms. We ask that
you bring these papers with you to your
child's appointment. Also bring the
names of any medications your child is
taking, a comfort item, i.e. toy, blanket,
etc.

If you have
dental insurance, please bring your card
with you. We gladly submit your insurance
claims to your insurance on your behalf.
Your portion of the bill is expected at the
time of service. If you do not have dental
insurance, payment is due at the time of
service.
